MEDICAL CONDITIONS/MEDICATIONS

If your child requires any of the following: Specialized Health Care Services, Medications, Medical Treatments, or has any health equipment needs during the school day (including bus transportation), i.e. crutches, walkers, wheelchairs, casting devices and/or interim medical testing devices: i.e. cardiac monitors/EEG/etc. Please contact your school nurse prior to the start of school and/or return to school, so that a plan of care can be put in place for your child's safety and well-being. Please click the Medical Forms link below for medication and care plan forms for your physician to fill out. If you are unsure of which forms you need to have completed, please call the school nurse to discuss your child's medical needs.

MEDICATIONS:
Any medications, both prescribed and over the counter, to be taken during school hours require both physician and parental approval prior to their administration. ALL medications must be brought to school by the parent or guardian in their original container, along with the completed medication form(s). Prescriptions must be under a current medication label and not expired. All scheduled drugs (ADHD, pain, anxiety, etc.) must be presented, counted and documented by the parent and nurse or their designated appointee. All medications must be pick up at the end of every school year.

Students who do not provide documentation of receiving the required immunizations on the first day of school will be sent home until documentation is provided. Days missed due to not having the required immunizations are considered unexcused absences.

** Homeschool/Virtual students who participate in WVSSAC sanctioned sports must provide a copy of the required school immunizations prior to the 1st practice/season start.

HEALTH/DENTAL CHECK FORMS BY AGE GROUP -

A current Health Check and Oral Screening Form is required at/or before the beginning of each school year for the following students: Pre-K, Kindergarten, 2nd, 7th and 12th grade students.
